utf-8+names: counter proposal from Rick Jelliffe

Taking advantage of a non-specification in XML 1.0, Rick Jelliffe has made a counter proposal to the "utf-8+names" Internet Draft which stays compatible with XML 1.0 and its existing encodings.
Since parsers are left free -in non validating mode- to do what they like with "Unexpanded Entity Reference Information Items", a simple SAX feature could be implemented to treat such references as references to HTML or MathML entities.
